For today
For the decades ahead
The goal
From a doctor who really knows you.
The problem
A handful of measurable risk factors drive many of the leading causes of death.
Many of these diseases silently progress for decades before ever showing their first symptom.

Membership isn't health insurance and doesn't satisfy any federal requirement to maintain it. We don't bill insurance for membership services, and we recommend keeping coverage for care outside the membership.
Who can join
Adults 18 and older, located in Texas, Florida, North Carolina, or Ohio at the time of each visit.
Not available through Caventa
Medicare patients (Join the waitlist), Medicaid patients, children under 18, pregnancy care, controlled substance prescriptions, in-person exams, and emergency care.
Common questions
Sign up online in just a few minutes or schedule a free meet and greet first to learn more and answer any questions you may have about the practice. Once enrolled, you can schedule your first visit where you'll review your history, medications, and goals with your doctor.
Adults 18 and older. You'll need to be physically located in Texas, Florida, North Carolina, or Ohio at the time of each visit, so we can't hold visits while you're traveling elsewhere. Labs are done through Quest Diagnostics, so you'll also need access to a Quest location or at-home collection. Each adult needs their own membership. We're unable to see patients enrolled in Medicare or Medicaid.
Not at this time. Caventa doesn't currently see patients enrolled in Medicare, including Medicare Advantage. If you enroll in Medicare while you're a member, your membership will need to end, and we'll help you transfer your care. You can join our Medicare waitlist to hear if this changes.
Membership is $150 a month, with no enrollment fee. It covers all of your care with your doctor: video visits, messaging, chronic condition management, new medical problems, longevity care, prescriptions, referrals, prior authorizations, and ordering your labs and imaging and reviewing the results with you. There are no copays or per-visit fees.
Not included:
No. Membership is paid directly to the practice, and we don't bill insurance for services covered by membership. That's by design. Billing insurance adds significant cost and administrative work, and it shapes how care is delivered, from how long visits last to what can be handled by message. Without it, your doctor is able to provide more attentive care to each patient by having fewer total patients and being more accessible. Lab tests are separate: Quest can bill those to your insurance if you choose (see “How do labs work?”).
Membership is not health insurance and doesn't replace it, and it doesn't satisfy any federal requirement to maintain health insurance. We recommend keeping health insurance for care outside the membership, such as labs, imaging, prescriptions, specialists, hospital care, and emergencies.
Yes. As of January 1, 2026, federal law treats direct primary care membership fees as a qualified medical expense, so you can pay your membership with HSA funds. You can use your HSA debit card, or pay with another card and submit your receipt to your HSA administrator for reimbursement. We can't give tax advice, so check with your HSA administrator or tax advisor if you have questions about your specific situation.
Yes, anytime, with no cancellation fee or penalty. Just send us a message through your patient portal or by email. Your doctor will continue to care for you for 30 days after we receive your request, and no further membership fees will be charged. Fees for a month that has already started aren't refunded.
Whenever something comes up, you can schedule a visit online, message your doctor directly through your secure patient portal, or call the office and leave a message. Messages are typically answered quickly during regular business hours, and video visits can usually be scheduled promptly. When needed, visits can often be arranged outside regular business hours as well.
Your doctor can help with many urgent concerns and may respond outside regular business hours, but after-hours responses aren't standard or guaranteed. If something can't wait for a reply, don't wait on a message or voicemail; seek care in person. For emergencies, call 911 or go to the nearest emergency room.
Caventa is virtual only. Many concerns can be handled well by video, but some can't be safely evaluated without an in-person exam, so you should expect that an in-person visit will sometimes be needed. When that happens, we'll direct you to an appropriate local option, such as urgent care or a specialist. In-person care is not included in your membership and is billed through your insurance or paid yourself at separate cash rates. For emergencies, call 911 or go to the nearest emergency room.
We offer a yearly preventive review by video and complete most paperwork as part of your membership. Forms that require a hands-on exam, such as some sports, employment, or DOT physicals, need an in-person clinician, so we can't complete them.
Caventa is designed to be your primary care practice. Depending on your insurance and needs, though, keeping another doctor can make sense in two situations:
If you need a specialist, your doctor can place a referral and send along your relevant records if needed. Specialist visits aren't included in your membership and are billed separately, usually through your insurance. If your plan requires referrals from an in-network doctor, see the question above.
Caventa doesn't currently accept patients enrolled in Medicare or Medicaid, and we don't provide care for children under 18, pregnancy care, or emergencies. We also don't prescribe controlled substances.
Because care is virtual, some conditions are better handled elsewhere. These include problems that need a hands-on exam, testing, or a procedure that can't be done by video (such as stitches, a pelvic exam, or a biopsy), and conditions that need specialized management beyond primary care.
Your doctor will decide what can be safely handled through telemedicine and you can schedule a free meet and greet to ask whether we treat a specific condition or prescribe a specific medication. If something falls outside our scope, we'll let you know and can point you toward the right kind of care.
Your lab tests are drawn and run by Quest Diagnostics, not by Caventa. For each lab order, you choose how to pay:
Imaging isn't included in your membership, but your doctor can order it and will review the results with you. You can take your order to any facility you choose and use your insurance. Coverage depends on your plan, and some tests, like an MRI or CT, may need prior authorization first. If you'd rather pay cash, we can send your order to a facility in the Green Imaging network, which offers upfront cash prices and bills you directly.
Prescriptions are sent to the pharmacy of your choice. Prescribing is included in membership; you pay for the medication itself through insurance, at the cash price, or with a discount program like GoodRx.
We prescribe GLP-1 medications when they're medically appropriate as part of a weight or metabolic health plan. We use FDA-approved GLP-1 medications and don't prescribe compounded copies of them. Medications are paid for at the pharmacy, not through your membership, and insurance coverage varies widely by plan. If your insurance doesn't cover them, we can help you look into manufacturer cash-pay options.
More broadly, we prescribe FDA-approved medications from established brand-name and generic manufacturers. We don't prescribe compounded copies of commercially available drugs, investigational drugs that haven't been FDA-approved, or anything from unregulated “research” sources. When a compounded medication is medically necessary, such as a dose or form that isn't made commercially, we use licensed compounding pharmacies that source their ingredients from FDA-registered suppliers.
No. We don't prescribe any controlled substances, and we can't take over an existing prescription for one, including short-term or bridge refills. This includes:
The same applies to any medication classified as controlled under state law. We also don't provide medical cannabis certifications or ketamine treatment. For some conditions, non-controlled treatments may be an option we can discuss.
Testosterone is a controlled substance, so we don't prescribe it in any form, whether for low testosterone or “hormone optimization.” We can evaluate symptoms, order testing, and refer you to a specialist if treatment may be appropriate. Menopause hormone therapy, such as estrogen and progesterone, is different: it isn't a controlled substance, and we can prescribe it when appropriate.
